Ep. 1: Overall Survival Updates in EGFR-Positive NSCLC: MARIPOSA and FLAURA2

Joshua Sabari, MD, of NYU Langone Health, welcomed expert oncologists to discuss advances in EGFR mutation-positive non–small cell lung cancer (NSCLC) at a Clinical Insights session coinciding with the IASLC/ASCO 2025 North America Conference on Lung Cancer, held in Chicago, Illinois. 

Panelists Danny Nguyen, MD, of City of Hope; Edgardo Santos MD, FACP, FASCO, of the Oncology Institute of Hope and Innovation; Eric Singhi, MD, of The University of Texas MD Anderson Cancer Center; and Alexander Spira, MD, PhD, FACP, FASCO, of NEXT Oncology, examined overall survival data from MARIPOSA (amivantamab plus lazertinib) and FLAURA2 (osimertinib plus chemotherapy).

Insights From the Panel on Emerging Data

Dr. Nguyen emphasized the significance of the 2025 data, highlighting that prior updates focused on progression-free survival. Now, with overall survival (OS) evidence, combination therapies show clear benefits over single-agent osimertinib. Dr. Spira reinforced that the findings validate early combination therapy as a clinical strategy.

Dr. Sabari noted that combining amivantamab, a bispecific antibody targeting EGFR and MET, with lazertinib, a third-generation EGFR inhibitor, represents a nonchemotherapy strategy designed to overcome resistance patterns. Dr. Santos explained that MARIPOSA’s efficacy stems from targeting MET amplification and other EGFR-mediated resistance pathways.

The panelists discussed potential reasons for the superior outcomes. Dr. Singhi suggested that immune modulation by amivantamab may contribute to extended survival. Dr. Nguyen pointed out that up-front therapy targeting both systemic and central nervous system disease might help prevent resistance.

Clinical Implications for Frontline EGFR Therapy

The panel agreed that single-agent osimertinib now has a limited role. Dr. Spira advocated for escalating therapy in most patients, reserving monotherapy for select cases involving frailty or logistical challenges. Dr. Santos emphasized the need to integrate patient preferences through shared decision-making.

Concluding the first segment, Dr. Sabari highlighted that MARIPOSA and FLAURA2 are shaping modern EGFR-positive NSCLC care. As OS data mature, clinicians anticipate continued refinement of combination strategies to maximize long-term patient benefit.
