Ep. 5: CNS Activity and Sequencing Strategies for Datopotamab Deruxtecan in EGFR-Mutant NSCLC

CNS Activity With Datopotamab Deruxtecan

Early signals from single-arm and randomized studies indicate that patients with brain metastases experience progression-free survival comparable to those without CNS involvement—an encouraging finding in a population historically linked to worse outcomes. Subset analyses have supported this observation, with intracranial responses exceeding 30% reported in patients with untreated brain metastases. 

The panel emphasized that symptomatic or high-risk brain metastases should still prompt early involvement of radiation oncology. However, the emerging CNS activity associated with Dato-DXd suggests that some patients may maintain intracranial control while receiving systemic therapy.

Clinical Perspective and Patient Education

This panel discussion consistently reinforced the importance of communication and multidisciplinary care. Education, close monitoring, and dose modifications when needed can effectively manage adverse effects such as mucositis, ocular toxicity, and rare interstitial lung disease.

With appropriate expectations and supportive care, patients are often able to tolerate Dato-DXd well and derive meaningful clinical benefit.
