Combination therapies have driven improvements in overall survival, shifting greater focus toward mitigating adverse events. Joshua Sabari, MD, of NYU Langone Health, led the discussion, highlighting the importance of patient-centered strategies to maximize therapy adherence and quality of life.
Clinical Implications
Danny Nguyen, MD, of City of Hope, explained that proactive strategies, including premedication with dexamethasone and split dosing, reduced infusion-related reactions from 66% to 22% in MARIPOSA. Subcutaneous formulations are expected to further minimize infusion-related concerns.
EGFR-mediated rashes remain common. Edgardo Santos Castillero, MD, FACP, FASCO, of the Oncology Institute of Hope and Innovation, described the proactive COCOON regimen using antibiotics, moisturizers, and sun protection, reducing grade 2 or higher rashes to 4.3%. Education and multidisciplinary nurse involvement ensure that patients manage side effects effectively.
Venous thromboembolism rates have declined significantly with the use of prophylactic anticoagulation. Alexander Spira, MD, PhD, FACP, FASCO, of NEXT Oncology, emphasized routine monitoring and early intervention, noting that prophylaxis is particularly effective with subcutaneous therapies.
Edema and hypoalbuminemia present ongoing challenges. Eric Singhi, MD, of The University of Texas MD Anderson Cancer Center, recommended lifestyle modifications, leg elevation, and supportive measures, emphasizing that dose interruptions don’t compromise progression-free survival.
Practical Considerations
The panel agreed that early recognition, proactive management, and patient education are critical. Dr. Sabari highlighted that these approaches allow patients to stay on therapy longer while maintaining efficacy.
The MARIPOSA experience demonstrates that adverse events, once a barrier, can now be effectively managed. By integrating proactive strategies, clinicians can help preserve survival benefits while maintaining quality of life.
